Service requests
Application has to be submitted in person, by post or by email (signed with secure electronic signature) or in an e-service (select the application type for receipt of the dose passbook).
The application has to include:
1) name, surname, personal identity number of the worker to whom you are requesting a dose passbook,
2) the postal address to which you want to receive the passbook,
3) contact details (telephone number, e-mail) in order to be able to contact the applicant.
The same application shall be completed and submitted in order to receive a passbook already in the archive.
Issuing a passbook is a free service.
In accordance with Regulations No. 1284 "Procedures for Control and Registration of the Exposure of Workers" individual ionising radiation dose passbook has to be received for worker who performs activities with ionising radiation sources. Information to be included in the dose passbook and the procedures for completing it are laid down in Chapter VII of Regulations No 1284.
If the worker works in more than one workplace?
The passbook shall be stored by the radiation protection officer at the time when the worker performs operations in the operator-controlled area. If a worker performs work with sources of ionising radiation at several operators, he or she shall inform the radiation protection officer of other jobs in order to ensure correct completion of the information in the passbook. The radiation protection officer shall issue the passbook to the worker for the time of performing the mandatory health check, as well as if the worker needs to fill out a passbook with another employer (operator).
If the passbook was filled or damaged?
The operator shall transfer the invalid or filled-in passbook to the Radiation Safety Centre of the State Environmental Service, as well as inform immediately regarding each case of loss of the passbook. The Radiation Safety Centre of the State Environmental Service shall issue a new dose passbook within 10 working days after receipt of the unused or filled passbook or receipt of information regarding loss.
If the worker stops working or does no longer work with sources of ionising radiation?
Upon termination of the employment relationship with the operator (employer), the dose passbook shall be transferred to the Radiation Safety Centre of the State Environmental Service, accompanied by a completed submission regarding the transfer of the dose passbook to the archive.
Information on ionising radiation doses received by the worker shall be kept by the operator and the Radiation Safety Centre until the worker reaches or would have reached the age of 75, but not less than 30 years after the work in question has been carried out.Receipt of services
The individual ionising radiation dose passbooks are sent by post to the address indicated in the application.
